A summary of benefits and premium levels is listed on the Individual Plan application. The
Individual Plan is an affordable coverage for your dental needs.
The coverage provides an initial exam at no charge and
up to a 40% discount on a majority of other dental services. These
services, including restorations (fillings), extractions, root canals,
crowns and dentures, are discounted off the Usual, Customary and
Reasonable (UCR) charges that non-insured patients pay.
